We are located on the third floor of the Providence Professional Plaza. Convenient parking is available in the parking structure located on Hoyt Street between Providence Portland Medical Center and Providence Professional Plaza. Free valet parking is available on Hoyt Street just under the sky bridge near the main entrance to Providence Professional Plaza.
New Patients
Please arrive at least 15 to 20 minutes prior to your first appointment. Please bring the following information with you to your appointment: completed paperwork, insurance card, photo ID and any co-pay.
Established Patients
Please arrive 10 minutes early to update any necessary paperwork before your appointment. Please bring the following information with you to your appointment: insurance card, photo ID and any co-pay.
